Designee:
A person appointed by the performance improvement participant to aid in the patient data collection process. A designee may or may not be a healthcare professional, as long as the person is familiar with the medical charts and medical terminology.
Concierge:
A service provided by Med-IQ to help make participation in performance improvement initiatives as convenient as possible for healthcare professionals. Participants can reach Med-IQ’s concierge professionals by phone at 866 858 7434, fax at 866 419 5789, and e-mail at concierge@med-iq.com.

Stage A Instructions
Step 1: Enter Patient Data
- Complete a self-assessment form. This must be completed by the participating healthcare professional prior to entering patient chart data.
- Complete a separate patient data form for each of the 10 patients with CRC you have selected. These forms can be completed by the participating healthcare professional or by a designee.
• Access ICD-9-CM codes for help identifying patient charts.

Step 2: Assess Your Current Performance
Review your My Performance page to find automatically generated reports summarizing how your data compare with current clinical practice standards, evidence-based recommendations, and data submitted by your peers.
Note: Your reports will generate immediately if you enter your CRC patient data online. If you submit your CRC patient data by mail or fax, your reports will appear on the My Performance page approximately 2 business days after your data are received by Med-IQ. If you do not have time to review your reports online, ask your concierge professional to e-mail or fax your reports to you.
Step 3: Select an Area to Improve
Review your reports and select one or more areas for improvement.
