Step 1: Enter Patient Data

1. Complete a 1-page self-assessment form. (This must be completed by the participating healthcare professional.)
2. Complete a 1-page data collection form for 20 diabetes patients. These forms can be completed by the participating healthcare professional or a proxy.
   
   Submit your diabetes patient data in one of 3 ways:

Step 2: Assess Your Current Performance

Review your My Performance page to find automatically generated reports summarizing how your data compares with national benchmarks, performance guidelines, and data submitted by your peers

Note: Your reports will generate immediately if you enter your diabetes patient data online. If you submit your diabetes patient data by mail or fax, your reports will appear on the My Performance page 2 business days after your data are received by Med-IQ. If you do not have time to review your reports online, ask your concierge professional to e-mail or fax your reports to you.

Step 3: Select an Area to Improve

Review your reports and select one or more areas for improvement
