Step 1: Participate in a Diabetes Educational Activity

Participate in one or more brief educational activities

Step 2: Plan Your Performance Improvements

Submit a brief plan indicating how you intend to improve your practice

Step 3: Implement Your Improvement Plan

Over the next 3 months, implement your improvement plan with your diabetes patients

Step 4: Attest to Implementing Your Plan

After 3 months of implementing your improvement plan, complete a brief form indicating how your diabetes practice changed

Stage B Complete: Redeem 5 AMA PRA Category 1 Credits™
