Stage C Instructions

15 minutes each form

Step 1: Enter Patient Data

Complete a data collection form for 10 patients with or at risk of osteoporosis. Please note: the patient visits from which you collect data at this stage must have occurred after you submitted your improvement plan. These forms can be completed by the participating healthcare professional or by a designee.

Step 2: Assess Your Performance Improvements

Review your My Performance page to find automatically generated reports summarizing how your Stage C data compare with your Stage A data, national benchmarks, performance guidelines, and data submitted by your peers.
Note: Your reports will generate immediately if you enter your osteoporosis patient data online. If you submit your osteoporosis patient data by mail or fax, your reports will appear on the My Performance page approximately 2 business days after your data are received by Med-IQ. If you do not have time to review your reports online, ask your concierge professional to e-mail or fax your reports to you.

Step 3: Evaluate the Program

Complete an evaluation form.

Stage C Complete: Receive 10 AMA PRA Category 1 Credits

Accreditation/Designation Statements: Med-IQ is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Med-IQ designates this activity for a maximum of 20.0 AMA PRA Category 1 Credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity.
